The day after we had the spurious record closed, a new record was created.
This is causing a problem, but I've no idea why or where this record originated.
I've reached out to my database contact to ask if he can trace the record.
What I may have to do is completely remove all the TV package from your account and close any TV references. When these are closed, re-add the TV package and let this re-provision through the system.
I'm just waiting for clarification from my colleague before proceeding down this route.
