My screen looks a little different to what you described.
When I click Internet Connection I get a summary of my connection and an Edit button. When I click Edit I get a long screen as you described, the only field that matches Name is looks to be a connection name field (see screenshot). I've completed this field as you said and applied then reset my TV my TV box. It's made no difference.
When I try to view any on demand programmes everything is fine but when using and Sky Sports channels the screen goes blank for a while and then pops up the error IPC6023
